Advanced Directives for Health
An Advanced Health Care Directive is a legal document where you state
your wishes regarding your health care if you're not able to make the decisions yourself.
This is also called a "living will," or a "durable attorney for health care." Everyone,
regardless of age, should have one of these filled out.

The Veterans Service Office provides veterans and their dependents and/or survivors with information regarding veterans benefits, and assistance with filing benefit claims, waivers and appeals, in order for them to obtain and maintain all available benefits and entitlements for which they are eligible.
The major categories of benefits and services consist of Disability Benefits, Dependents and Survivor Benefits, Education Benefits, Vocational Rehabilitation, Burial Benefits, Health Care, access assistance, Veterans Homes, Home Loans, and Life Insurance.
The VSO maintains contact with networks with the U.S. Dept. of Veterans Affairs, the Calif. Dept. of Veterans Affairs, other County Veterans Service Offices throughout the State, local veterans' groups and service organizations, and other community resources available to veterans, dependents, and survivors. Home Based Primary Care
This service provides primary home health care, including a doctor, nurse, social worker, dietician and therapist. The teams makes home visits to assist the patient and family care for the veteran's needs and cope with present illness. The team DOES NOT PROVIDE: home health aide, homemakers, housekeeping or daily nursing care. The program is available to patients living within a 30 mile radius of the Martinez Outpatient Clinic. 925-372-2131
Veteran's Assistance Benefits
"Aid and Assistance Benefit" this benefit helps pay for "Out of Pocket Living Expenses," and both home care and assisted care living qualify for this benefit. Both veteran's and spouses may qualify for this benefit. The benefit is paid directly by the V.A. to the veteran. For more information:
